One-to-One Support Protocol

Introduction

The One-to-One Support Protocol supports health and social care staff and care providers in arranging, monitoring, and reviewing one-to-one commissioned support in a consistent and person-centred way.

Why it matters

The aim is to ensure that support is:

  • the least restrictive option available
  • the least intrusive approach possible
  • still fully meets the individual’s assessed needs

Who this guidance applies to

This guidance applies to care that is:

  • funded by Adult Social Care (ASC) through North Yorkshire Council
  • eligible for Continuing Healthcare (CHC)
  • jointly funded by North Yorkshire Council and Humber and North Yorkshire Integrated Care Board
  • supported under Discharge to Assess (D2A) pathways
  • entitled to Section 117 Aftercare

Launch of the protocol

The One-to-One Support Protocol was launched in October 2025 to the care provider market and North Yorkshire Council and Integrated Care Board assessors and practitioners. 

A joint session was held with representatives from North Yorkshire Council and the Humber and North Yorkshire Integrated Care Board, who presented the protocol and its purpose. The recording of the session can be watched in the video below:
